Kentucky IRP Base Requirements for First Time Applicants

*Motor Carrier as Registrant*

- Completed Schedule C Application TC 95-303.
- Schedule B TC95-303.
- Apportioned Certificate (Form TC96-81) from County Clerk (applicant must apply for title and obtain certificate prior to filing for IRP.)
- Proof of Liability Insurance.
- Stamped Form 2290 (Issued by the Internal Revenue Service) for vehicles 55,000 pounds combined gross weight and over for the current tax period. This is for vehicles that have been owned for 60 days or more.
- Updated Federal MCS-150 Form (USDOT Application).
- 3 forms of proof of physical Kentucky address for established place of business verification. Examples of these are articles of incorporation, copies of Kentucky vehicle title in company's name, income tax return in companies name, phone or utility bills in company's name showing physical address.


*Owner Operator as Registrant*

- Copy of lease agreement.
- Lease company's DOT number and tax ID number.
- 3 forms of proof of physical Kentucky address verification. Examples of these are Kentucky CDL, copy of Kentucky vehicle title in applicant's name, income tax return in applicant's name, phone or utility bill in applicant's name showing physical address.
- Schedule B and C, copy of Apportioned certificate, proof of liability insurance, stamped Form 2290 as described above.
