- Schedules
A, B & C. These forms are sent to you 6 weeks prior to
your expiration date. They must be completed and signed.
- Apportioned Certificate (issued at your local county
clerk's office).
- Proof of insurance.
- Stamped Form
2290 (Issued by the Internal Revenue Service) for vehicles
55,000 pounds combined gross weight and over for the current
tax period. This is required for vehicles that have been owned
for more than 60 days.
- If the vehicle is leased on to a motor carrier, a copy of
the lease agreement must be included each year.
